The shape of `sp^3 d` hybridized molecule is trigonal bipyramidal. The three planar triangular hybrid orbitals may be assumed to be formed from one s- and two p-orbitals. So the remaining `p_z` - orbital may combine with` d_(z^2)` (aligned along z- axis) to form two axial hybrid orbitals that result in the formation of `sp^3 d` hybridized orbitals.
